Community Partners of WNY

Community Partners of WNY is a network of more than 100 health, human service, and educational organizations; the Catholic Health System plus five community hospitals; and over 1,000 physicians from across the region that will focus on transforming the delivery of healthcare in Western New York. This community-wide effort is governed by a representative board established by the lead organization, Sisters of Charity Hospital, and supported by the project management team at Catholic Medical Partners. Our goals are to improve clinical care and service to the Medicaid population and to achieve a measurable reduction in the burden of illness for our population, while achieving the New York State target of a 25 percent reduction in avoidable hospital use over a five-year period. Community Partners of Western New York will develop our human resource capabilities by training and empowering our workforce to use best clinical and operational science, health information technology, and a proactive approach to patient care in order to achieve our goals.

CPWNY is pleased to announce that HEALTHeLINK consent forms are now available in five additional languages! Languages available include: English, Spanish, Arabic, Bengali, Burmese, Karen, Nepali, Polish, Russian, and Somali.
