Healthy Community Alliance – join the CBO Consortium!

The Healthy Community Alliance has been awarded with funding from the NYS Department of Health to assist Community Based Organizations (CBOs) with the development of long term sustainability plans. CBOs that have addressed Social Determinants of Health for over two years are invited to join the Rest of State CBO Consortium. While technical assistance webinars have begun (a list of those can be found here *updated schedule posted 6/24*), they are still accepting CBOs into this consortium to participate in this support and plan for their futures. More information on the CBO Consortium and key benefits of membership can be found on their benefits and key messages pages.

To register with the CBO Consortium, the CBO Agreement form must be completed and submitted to Tony Sanfilippo (contact information on the flier below), and the CBO applying for membership must fulfill the requirements listed on the flier. If your organization is interested in participating in the next regional meeting on June 27 for the CBO Consortium, Tony Sanfilippo is also the contact to register for the call. The presentations from the first meeting on May 30 are also available here.

CBOs that do not meet the criteria can also register using the affiliate CBO form organizations and are invited to participate in the Consortium, but will not receive incentive payments to do so.

Live Well WNY

Live Well WNY is a movement led by the Population Health Collaborative and funded by the Health Foundation for Western & Central New York. The program seeks to address three behaviors that lead to four chronic conditions that account for over 50% of our premature deaths. These three behaviors are smoking, inactive lifestyle, and unhealthy diet.

Any organizations or programs that are interested in supporting this work by listing themselves as a resource for community members working on these issues, please register on the WNY Healthy Communities Coalition webpage.

Also, please feel free to join the WNY Healthy Communities Coalition on June 28, 2019 at AcQua on the Buffalo waterfront for a chance to meet and speak with others doing similar work around healthy living. For more information and to register for this free event, please sign-up on their eventpage.
