Community Partners of WNY is a network of more than 100 health, human service, and educational organizations; the Catholic Health System plus five community hospitals; and over 1,000 physicians from across the region that focus on transforming the delivery of healthcare in Western New York. This community-wide effort is governed by a representative board established by the lead organization, Sisters of Charity Hospital, and supported by the project management team at Catholic Medical Partners. Our goals are to improve clinical care and service to the Medicaid population and to achieve a measurable reduction in the burden of illness for our population, while achieving the New York State target of a 25 percent reduction in avoidable hospital use over a five-year period. Community Partners of Western New York will develop our human resource capabilities by training and empowering our workforce to use best clinical and operational science, health information technology, and a proactive approach to patient care in order to achieve our goals.
The Community Partners of WNY Performing Provider System is being supported by funding through the New York State Delivery System Reform Incentive Payment program (DSRIP). The DSRIP program, which was established in 2014, will allow the state to reinvest $8 billion in federal savings generated by the Medicaid Redesign Team reforms. Those funds are being used to address critical issues across the state and encourage comprehensive reforms. The DSRIP program promotes community-level collaborations and focuses on reforming the healthcare delivery system.
