The Community Partners of WNY PPS brings together clinical partners with more than 100 health and social service organizations from across Western New York, behavioral health providers, and other community based organizations. This wide-reaching collaboration of providers from every sector of the healthcare community is tasked with creating strategies to address ten important healthcare initiatives in our community, as identified by the New York State Department of Health. These initiatives are:

  1. Create Integrated Delivery Systems that are focused on Evidence-based Medicine/Population Health Management
  2. Emergency Department triage for at-risk patients
  3. Care transitions model to reduce 30-day readmission for chronic health conditions
  4. Expand usage of telemedicine in underserved areas to provide access to otherwise scarce services
  5. Integration of primary care and behavioral health services
  6. Cardiovascular Health- Evidence-based strategies for disease management in high-risk:affected populations (adult only)
  7. Increase support programs for maternal and child health through the Nurse Family Partnership Model
  8. Integration of palliative care into the PCMH model
  9. Promote mental, emotional and behavioral (MEB) well-being in communities
  10. Promote tobacco use cessation, especially among low SES populations and those with poor mental health
